What to Bring

Important: The Live Scan form is provided by the requesting agency (school, employer, licensing agency, etc.). ACCU-Print does not provide this form.

Accepted Primary Forms of ID

You must present one of the following:

  • State-issued driver’s license or ID card
  • U.S. Passport or Passport Card
  • Federal Government PIV Card
  • Uniformed Services ID Card
  • Department of Defense Common Access Card
  • Foreign Passport with appropriate immigration documents (e.g., Form I-94)
  • USCIS Permanent Resident Card (I-551)
  • USCIS Employment Authorization Card (I-766)
  • Federal, state, or local government agency ID with photograph
  • U.S. Coast Guard Merchant Mariner Card
  • Canadian driver’s license

If You Do Not Have a Primary ID

You must provide:

  • At least two secondary forms of identification
  • AND
  • At least two supporting documents (listed below)

Secondary Forms of Identification

  • State-issued birth certificate
  • U.S. Tribal or Bureau of Indian Affairs ID card
  • Native American tribal document
  • Social Security card
  • Court order (name change, adoption, divorce, etc.)
  • Government-issued marriage certificate
  • Consular Report of Birth Abroad
  • Draft record
  • School ID with photo (minimum age 7)
  • Certificate of Citizenship (N-560/N-561)
  • Certificate of Naturalization (N-550/N-570)

Supporting Documents

To verify secondary ID, you must provide at least two of the following:

  • Utility bill (showing address)
  • Voter registration card
  • Vehicle registration or title
  • Paycheck stub with name and address*
  • Public assistance card
  • Spouse or parent affidavit

*Financial information may be redacted.
