Accepted Forms of ID

Accepted Primary Forms of ID

Primary identification must be current, valid, and unexpired.

You may provide any one of the following:

  • State-issued driver's license or ID card
  • U.S. Passport or Passport Card
  • Federal Government Personal Identity Verification (PIV) Card
  • Uniformed Services Identification Card
  • Department of Defense Common Access Card
  • Foreign Passport with appropriate immigration documents (e.g., Form I-94)
  • USCIS Permanent Resident Card (I-551)
  • USCIS Employment Authorization Card (I-766)
  • Federal, state, or local government agency ID card with photograph
  • U.S. Coast Guard Merchant Mariner Card
  • Canadian driver's license

When using secondary identification, at least two of the following supporting documents are also required:

  • Utility Bill (showing address)

  • Paycheck Stub with Name and Address*
  • Public Assistance Card
  • Spouse or Parent Affidavit (PDF)
